Required Documentation
Please include documentation verifying your enrollment in a federal or state-recognized tribe, per the 1990 Indian Arts & Crafts Board (IACB) Act. Keep original documents for files and send photocopies. Please be advised that we cannot accept documentation that does not verify your Tribal Enrollment. (Documentation that verifies only your blood cannot be accepted.)

Verifying Documentation Accepted

Submit one of the following:
• A copy of your tribal enrollment card.
• A copy of a statement/form from your tribe or agency on official letterhead and signed by a senior tribal official or tribal enrollment specialist, clearly verifying your tribal enrollment in no uncertain terms.
• The IACB Act allows a non-enrolled Indian Artist to be certified, in writing, by the tribe of his/her descent as an "Indian Artisan" of that tribe�s descent. A copy of that certification will be accepted as documentation.
